Our practice is currently taking new patients,
We require a deposit of $150.00 for new patients
This fee applies to all new patients,
The second consultation and onwards will be as per our fee schedule.
Practice Forms:
As a new patient to the practice, it is important for your doctor to understand your past medical history so that they can begin to treat you in the best way for your health care needs.
You can help by writing down your past medical history, current medications and health concerns on the New Patient History form and bringing that along to your first appointment.
Transferring Your Medical Information
If you have access to your medical history, your new doctor may need a copy of your medical record from your previous GP practice.Transferring medical files is a standard procedure when patients change practices.We can assist you in arranging this transfer. Please note that it is common for to charge a small fee to cover administrative and postage. Your previous practice will inform you about any transfer fees that may apply.
